COMMUNITY STABILIZATION

Community Stabilization services are short-term and designed to support an individual and their natural support system following contact with an initial crisis response service or as a diversion to a higher level of care. Providers deliver community stabilization services in an individual’s natural environment and provide referral and linkage to other community based services at the appropriate level of care. The goal of Community Stabilization services is to stabilize the individual within their community and support the individual and/or support system during the periods 1) between an initial Mobile Crisis Response and entry in to an established follow-up service at the appropriate level of care 2) as a transitional step-down from a higher level of care if the next level of care service is identified but not immediately available for access or 3) as a diversion to a higher level of care.

MENTAL HEALTH SKILLS BUILDING

Mental Health Skills Building Support Service provides community based services, which includes training and support to enable individuals age 18 years and older with significant psychiatric functional limitations to achieve and maintain community stability and independence. AFTER SCHOOL PROGRAM

The After School Program provides daily therapeutic services during the school year to youth with significant behavioral and emotional problems in a setting that addresses behavioral problems. Our Qualified Mental Health Professionals (QMHP) furnish transportation for our young people from school to one of our New Life sites. On arrival, the youths receive healthy snacks. TDT IN SCHOOLS PROGRAM

New Life counselors work with school personnel to offer a Day Treatment Program designed to address behavioral issues and help students succeed in their school environment. Creating individualized goals for each student, counselors lead students from negative behaviors toward a future of positive outcomes. PARTIAL HOSPITALIZATION PROGRAMS

PHP – The New Life Behavioral Health Services Partial Hospitalization Program (PHP) uses proven, evidence-based best practices. This program can help people who are stepping down from inpatient care, providing structure and encouragement as individuals re-enter their family and community environments. This program can also be step up from outpatient care, with the goal of helping people avoid an inpatient psychiatric admission.How long a person stays in the program depends on the specific goals and treatment plan, but most participants are in the program for one to two weeks.PHP runs from 9 a.m. to 4 p.m., Monday through Friday.The backbone of PHP is therapy in small groups, with 8 to 10 participants. Individual counseling is also provided. Participants learn to build on their existing skills and develop additional coping strategies. It is a positive, motivating environment.

The following services are provided by our sister agency Behavioral Health Associates.

Community Stabilization
Our team of Qualified Mental Health staff help children, adolescents and adults who are experiencing an acute psychiatric crisis that places them at risk of hospitalization. This intensive program is short-term, up to 7-10 days.

Individuals must meet all of the following criteria to qualify for Community Stabilization Services:

  1. The individual has experienced a recent behavioral health crisis (within 72 hours of admission) or the individual is transitioning from a higher level of care and
    requires short-term support with identifying and/or engaging in the services necessary to maintain safety and stability in the community;
  2. Documentation indicates evidence that the individual meets criteria for a primary diagnosis consistent with the most recent version of the Diagnostic and Statistical Manual;
  3. There is evidence from the individual or collateral contact indicating at least one of the following is present: a. High potential for crisis-cycling without this support; b. Individual does not have the ability and/or the resources to support maintenance of safety and/or stability in the community until longer term services are available/accessible; Individual has been engaged in alternative crisis services or treatment and no longer meet criteria for those services but continue to require community stabilization support;
  4. The individual currently has moderate to high intensity behavioral and/or emotional needs and without intervention, will further interfere with their ability to function in at least one life domain: family, living situation, school, social, work or community.

Our staff work with individuals in their home and community to   stabilize their acute mental health needs at the earliest possible time and avoid psychiatric hospitalization. If the person served is a child or adolescent, it is crucial to the success of the program that at least one parent or guardian participates in services.

Outpatient Therapy

The  Outpatient treatment continuum of care includes individual therapy, play therapy, couples, family therapy, and psycho-educational group  therapy. Some of the issues that we address are: anxiety, depression, bipolar disorder, PTSD, ADHD, substance abuse, trauma, abuse and neglect, grief and loss, foster care and adoption, anger management difficulties, family/relationship conflicts, and behavioral difficulties. We offer personalized support for your unique needs. Psychiatry

In certain situations, medications are a helpful and appropriate approach to the diagnosis, treatment and prevention of mental, emotional and behavioral disorders. Our psychiatric providers can improve quality of life and may even be able to prevent certain crises from arising by promoting mental health stability. While medication is rarely our first line of treatment, our providers know that mental health concerns are legitimate medical issues, often best managed
with medication.
